Account Maintenance
Used to change or update name and address of a group annuity participant or to make corrections to participant information such as social security number.
This form is used to change or update the beneficiary information on a 457b annuity. It can be used for Governmental or Non-governmental contracts.
ERISA or Non-ERISA change of Beneficiary and Spousal Consent form.
This form is used for changes or corrections to the beneficiary information on a contract. (Not to be used for custodian owned contracts, irrevocable beneficiary changes, controlled payout beneficiary requests, or ERISA contracts).
This form is used to update an owner's or annuitant's contact information, including changes or corrections to a name, address, or phone number. Not to be used for ownership changes, which require an Owner/Annuitant Change form.

This form is to be used for the contract owner to authorize the release of contract information to a third party.
Loan
Loan Request and Loan Agreement for ERISA 403b Annuities. Use this form for ERISA cases except for Integrated Resources Contracts.
Loan Request and Loan Agreement for Non-ERISA 403b Annuities. Use this form for Non-ERISA cases except for Integrated Resources Contracts.
This form is used to request a Account Reduction loan, it includes both the loan request and the loan worksheet.
Transfer of Assets
Required to be returned with the application when requesting a transfer or rollover into a qualified retirement plan.
Required when initiating a partial 1035 exchange.
Withdrawals
This form is used to request a hardship withdrawal from a MetLife 401k plan that follows the 401k deemed or safe harbor hardship withdrawal rules and where MetLife has agreed to pay distributions and do tax withholding.
To request a hardship withdrawal from a 403b-TSA under the 01-01-09 403B regulation changes-can be used both ERISA and Non-ERISA Plans.
Used for a participant or alternate payee to request a distribution from a 403b ERISA annuity other than for a hardship or a systematic withdrawal.
A participant or alternate payee of a 457b plan (state or local government) can use this form to request a withdrawal from a MetLife annuity for reasons other than unforeseeable emergency.
This form is used to request an Unforeseeable Emergency withdrawal from the participants account. These withdrawals are subject to the Plan Provisions and approval.
A participant or alternate payee can use this form to request a distribution from a 403b Non-ERISA annuity. This form is not used for a hardship or systematic withdrawal.
Can be used for a participant or alternate payee in a 457b Non-Governmental plan to request a withdrawal from a MetLife Annuity.
